Medicaid Service Coordinator
A Medicaid Service Coordinator (MSC) works in partnership with a consumer and, if desired, his or her advocate, to create and maintain an individualized service environment (ISE). The MSC works with consumers to enable them to live and participate in the community according to their personal goals, preferences, and needs. The MSC plays a major role in the development of a consumer’s individualized service plan. The MSC is responsible for negotiating with all providers to ensure the consumer receives the most adequate and appropriate services.
1 year of professional OPWDD experience. Knowledgeable about OPWDD case management guidelines and services. All MSCs must be CORE Certified and complete the annual training requirements of 15 hours or more from OPWDD’s MSC Training Curriculum. Annual training requirements are needed based on anniversary date. Anniversary dates are based on date of employment with Agency.
Home Health Aide
It is the responsibility of the Home Health Aide to provide one-on-one supervision of patients in their home or other service environments by monitoring and reporting patient’s condition, providing support and personal services, and teaching patient and caregivers.
18 years of age or older. Certificate of training for Certified Home Health Aide from an approved NY State DOH or NY State Education Training School. CHRC Clearance. Successful completion of a competency evaluation. Ability to follow patient’s care plan
Direct Support Professional
Direct Support Professional performs a wide variety of routine tasks and activities in support of the full range of services provided to developmentally disabled individuals by the Agency. The tasks and activities performed depend upon the specific abilities of the individuals being served and the nature of the setting where the work is performed. These tasks involve encouraging, guiding and training individuals in developing daily living skills and habits, taking care of their personal needs, assuring community integration, ensuring the health and safety of individuals, and maintaining the service environment.
Direct Support Professional, under general supervision of clinical or higher level staff, provides direct care to developmentally disabled individuals in a residential and/or program setting; assists and motivates individuals in their care to attend to their personal daily living needs and to further develop the vocational, nutritional, social, and personal skills necessary to maintain or achieve the highest possible level of independent functioning in the least restrictive environment.
