BRIDGES TO HEALTH

Bridges to Health:

The Bridges to Health (B2H) Waiver Program is funded by OCFS and designed specifically for vulnerable children. B2H offers services not otherwise available in the community to children with complex medical conditions, and does so in the context of their often complicated family and caregiver network. By supporting children in foster care, community services are provided in the home or community. The B2H Waiver Program provides opportunities for improving the health and well-being of the children served, and supporting permanency planning.

The B2H Home and Community-Based Waiver Program consists of three waivers:
  • B2H for children with serious emotional disturbances
  • B2H for children with developmental disabilities
  • B2H for medically fragile children

The B2H Waiver Program is designed to recognize that children in foster care can have many caregivers involved in their lives. In the program, children are served in the least restrictive environment.

Unique aspects of B2H services:
  • Services are conducted in the home or community setting.
  • Provides supports for you, your child and others in your child’s life.
  • Participation is voluntary.
  • Families choose providers and work with case managers to choose services.
  • Supports permanency planning.
  • If eligible, services continue after child leaves foster care, until age 21.
Eligibility:
If your child meets the following criteria, talk with your case planner about getting your child enrolled:
  • Is in the custody of the Commissioner of the Local Department of Social Services (LDSS), Administration for Children’s Services (ACS), or Office of Children and Family Services (OCFS).
  • Has a diagnosis of Seriously Emotionally Disturbed, Developmentally Disabled, or Medically Fragile.
  • Is at risk of requiring an institutional level of care.

  • Day Habilitation
  • Pre-vocational training
  • Supported Employment Services
  • Family/Caregiver Supports and Services
  • Planned and Crisis respite care
  • Immediate Crisis Response Services
  • Crisis Avoidance, Management and Training
  • Special Needs Community Advocacy and Support
  • Intensive In-home Supports and Services
