Quality health care in the comfort of your own home
We provide home care services to help individuals with disabilities have the right support and assistance for them to stay independent and comfortable in their homes. Depending on the individual situation, we can arrange personal care services from a few hours a day to around-the-clock care.
Your physician requests Home Care prior to discharge from the hospital and determines your eligibility for Home Health Care services working with the Discharge Planner, Case Manager, or Community Agency. After receiving the referral, our intake Nurse meets you and your family to review your home care needs, medications and the supplies or equipment ordered by your physician.
We accept private pay, insurance and Medicaid. If you have any questions regarding this service, feel free to contact us.

What can you expect from home care services?
Nursing assessment
After your discharge from the hospital, our Agency Health Nurse visits you at home to complete your initial assessment visit. Your physician determines the type, frequency and duration of the home visits. Periodically, your physician reviews and changes care plan to ensure appropriate home health care services.
Home care coordination
Upon admission into our Agency, you are assigned a nurse coordinator who is responsible for integrating your services into a comprehensive plan of care.
Nursing supervision
Periodically, our Nursing Supervisor will visit your home to ensure that the home health care services ordered by the physician are properly implemented by our staff.
Personal Care Aide (PCA)
Our Personal Care Aide (PCA) services provide assistance with activities of daily living to individuals with special needs. Our highly trained and compassionate PCAs are committed to providing exceptional care and support, helping individuals to maintain their independence and quality of life.
Home Care Eligibility
Enrollment is open to all parties residing in New York City and Nassau. You must require services of any of the following:
Registered Nurse (RN), License
Housekeeper
Home Health Aide (HHA)
Medical Social Services
Personal Care Aide (PCA)
Homemaker
